Dispute of a loan fraudulently taken out in my name

Prepare a notice to a company about a loan agreement you did not enter into. Describe the circumstances and request verification and preservation of records. The letter does not replace reporting suspected crime or taking steps to protect your identity.

How to complete the document

  1. Use this template only if you did not enter into the agreement or authorise someone to do so. Dispute its formation rather than submitting a withdrawal notice.
  2. Verify the company’s official contact independently. Provide only information needed to identify the case; do not automatically attach a complete identity document copy.
  3. If you suspect a crime, report it to the Police or prosecutor. Consider promptly restricting use of your PESEL and reporting misuse of your identity document.
  4. If court papers arrive, observe their deadlines and seek legal help. A notice to the company does not replace procedural steps.

This template must be adapted to your circumstances. Before signing or sending it, remove uncompleted fields and check all amounts, dates and party details.
